Abstract: Many neurological diseases, including multiple sclerosis (MS) and dementia,
progress without apparent symptoms, with substantial damage accumulating before it is
detected clinically. Brain imaging, empowered by advanced analytics, offers
considerable opportunities to monitor this subclinical progression, accelerating
scientific discovery in clinical neuroscience and enabling precision care.
This talk presents a collection of work from the Computational Neuroimaging Research
Group at the Brain and Mind Centre to generate disease insights and improve the
management of chronic neurological disease. It traces the development of a series of
methods and AI models across MS and other neurological conditions, from research tools
to a routine clinical service now deployed in more than 200 clinics across Australia and
internationally, processing thousands of scans each month to support clinical
decisions. The talk also highlights the engineering challenges of implementing AI in
real-world clinical settings, including those that proved unexpected, and outlines the
methodological advances still needed, such as robustness across scanners and sites,
reliable uncertainty estimates, and individualised prediction of disease course.
